• Who owns or is responsible for it
• Recent activity, comments, updates, or movement
• A decision being made, moved, or delayed
• Waiting, stalled progress, or delay
• Ownership that is unclear or shifting
• Work repeating, reopening, or looping
• Escalation or intervention
